fix: [approval] 재직증명서 DOCX 생성을 PhpWord 직접 생성으로 변경

- 외부 템플릿 파일(employment_cert.docx) 의존성 제거
- PhpWord로 테이블/텍스트 직접 생성하여 서버 배포 시 템플릿 누락 문제 해결
